Oncolife: Survivorship Care Plan
OncoLink has created an individualized plan of care for cancer patients based on recommendations from the Abrahamson Cancer Center of the University of Pennsylvania. It encourages patients to review their plan with their healthcare team to further assess their risk and become active participants in their follow up care.
